With a skills matrix! Here's how it works. Your team leaders record the necessary

skills that should exist in order to deliver a high quality output (be it a product or service). Then by recording the available skills of the team members, this solution produces a skills 
matrix outlining skills strengths and weaknesses (gaps and opportunities).

This process provides your team leaders, managers and trainers with a clear, visual heat-map of skills within their team.

List all the people within your team down the left-hand side of the matrix.

How to determine capability level?

Taking inspiration from the Toyota Production Systems (TPS) and years of interaction with thousands of companies, we believe the below to be a great place to start.  Its crucially important that all team leaders and team managers work to the same scoring methodology in order to create true consistency. The weightings are used to create a total proficiency score at individual and process level, helping you to understand your teams true capability.
